Maybe it was in Word Perfect, but I know at one time I created custom

Up and including to Word 2003, Tools | Customize | Toolbars is your friend. In Word 2007, there simply are no toolbars anymore. The whole new Ribbon UI is a big context menu, and the means to adapt it are few or only open to XML wizards (or 3rd-party tools).
