Default dictionary-style headers

I have created the dictionary-style headers; however,
when I make corrections, some of the headers are blank.
The paragraphs also have different styles and formatting.

You need to consistently use heading styles to make the StyleRef field work.
It is this field that is used for dictionary style headers.
