Default Document Map

Some of my Heading 3 entries show up with the outline number (ie, x.y.z), but
others don't. I've tried to copy/paste formats with no success.

Any ideas?

If you've set up your outline numbering correctly (see
http://www.shaunakelly.com/word/numb...Numbering.html) and linked
each numbering level to a style, then Ctrl+Q should restore the proper
numbering.
