Have they all had the appropriate Style applied to them?

Only Word's built-in heading styles will show up as "Headings." Any numbered
paragraph will show up as a "Numbered item." Anything else will have to be bookmarked by the user and will show up under "Bookmarks." -- Suzanne S.
