Insert { FILENAME } and { CREATEDATE } fields into the document.

But note that at the time of creation the document will have neither
filename nor path. These are only provided when the document is saved. Before that the document will have a temporary name based on the type of document - eg Document1 Note also that {Filename} produces only the filename.
