I am trying to write a letter of gratitude to my mother for all the years of
dedication and not sure if there is a template on which to follow.

One way to find template is to visit Office Online template page at
http://office.microsoft.com/en-us/te...s/default.aspx and type "Thank you"
to the search [Templates] box and click "Go"

I think your mother would be very unhappy if she thought you were sending
her a "form letter." If you can't express your gratitude in your own words,
you might do better to remain silent. g
