How can I link the table of contents to go directly to the page?

If you insert a TOC using Insert | Reference | Table of Contents, the pages
numbers are hyperlinked in all versions of Word, and the entire entry is
hyperlinked by default in Word 2000 and above.

And if you are inserting a TOC manually, or editing an existing one,
you'll need the \h switch in order for the entries to be hyperlinked
(only available to Word 2000 and later). For more, see:

This is true, but the \h switch is inserted by default in Word 2000, 2002,
and 2003, and in Word 2000 it can be removed only by hand-editing the field
(there is no option in the TOC dialog itself).
