Default making equal lines in a form

anyone that can help Please and THANKS
I create many forms and consistently have a problem getting my lines to come
out even.

for example
 vision ____________________________________
 hearing __________________________________
 motor ____________________________________
Note how the lines looke like they are different lengths... I cannot get
them to come out equal.

Any suggestions???

Default making equal lines in a form

If you're just creating a document rather than an actual form, I would
suggest using tabs with a line leader. Format | Tabs | Leader | Select 4 and
set the tab stop where you would like it to be.
