Default How do I set up user forms in Word?

I wish to set up a user form that the client cannot change the field
headings. Once the client completes the form he/she can then email it back to
me.

I don't think you want a User Form. That is a custom dialog box. What you
are talking about is what Word calls an "online form." Check this in help.
