Default Scanning an Entry Form to use as a Template

Help! What is the easiest way to scan an "Entry Form" as an exact copy
template and then use that template to enter Names/Adresses/Info ?? Thanks.

Depends what you mean by 'easy' ... least work now in setting up the form,
or least effort long-term in using the form?

If it's only a one-off, scan the form as for any other graphic. Then
superimpose transparent, borderless textboxes where you want the text.

If it's a form you will use repeatedly, forget scanning. Create a new
document that looks like the original, with formfields for text entry.

If you are going to use this a lot - either start from scratch as suggested
by Jezebel, or consider the Form building tool that comes with some versions
of Finereader OCR software.
