Has anyone been able to find the insert address icon in Word 2007 to add to
the Quick Access Toolbar?

Default insert address

Right click the QAT, choose Customize QAT, set Choose Commands From: to All
Commands. In that list, look for Address Book...

Since you said "Quick Access Toolbar" in your question, I mistakenly thought
you'd leap to the conclusion that its shorthand is QAT. This new
jargon/terminology is going to be a bit of a hurdle, I guess.
