Default Posting a document on a website and have it signed electronically

I work in HR for a company and we need employees to view policies and confirm
that they have received them. In the past we would print the policies and
have employees sign that they have read and received it.
We now have an HR internet site and I thought it would be a good idea to try
to do this electronically. I would like to post the document (word, pdf,
powerpoint) on the site and be able to track that employees have viewed the
policy by having them enter their employee number. Then I could track the
response by employee number.
Any ideas on how I could accomplish this?

Look into pdf forms created with Adobe Livecycle Designer, these forms can
send you an email confirmation of activity or use.

Of the several possibilities, that's hte best one. The MS apps will give
you problems one way or another which pdf will not.
