Default Data Form

Years ago I created a simple data form to enter the name and address
in a form letter. It works OK most of the time but occasionally one of
the line goes missing so I have to enter it again. The line that goes
missing is always either the zip code or the state and occasionally
both.

Any ideas?

Not without a LOT more information, such as what this "data form"
actually is and how it works. Plus the version of Word.
