Default Macro toolbars in word 2007

In word 2003 I have a template .dot file which contains macros and a custom
toolbar.
Any document which is based on this template then has access to the macros
and the toolbar

Basically how does this work in 2007 ?

If I use the dot template in 2007 it adds a new Add-ins tab to the ribbon
which is good, but I can see no way of editing the icons or entries etc as
you could in 2003

I can also find no way of putting an entry on to the add-ins tab

Default Macro toolbars in word 2007

If you can't do it programmatically (meaning using the CommandBars
thing), then you can't do it in 2007.
You can only add icons to the macros to your Quick Access Toolbar.
