Default TOC updating problems

I am using some if statements and the includetext fields to insert specific
files into a lager document. The problem is when the TOC updates it shows a
lot of blank entries. The new file entries are there, but the blank ones are
there too.
Wondering if anyone has seen that, and would know how to fix it.

The problem could be that you have empty paragraphs (i.e. paragraphs
consisting of a paragraph mark only) in your document and that some of these
paragraphs are formatted with the styles that are included in the TOC. I
suggest that you turn on formatting marks and check the document for such
empty paragraphs.

I can't find any blank paragraphs. Maybe the problem has something to do
with the field code. The files I am including contain certian headers.
Could this be causing a problem with the field code?

If you have inserted manual page breaks before any headings, they will be
formatted in the heading style and will appear as blank entries. Instead of
using a manual page break, format the heading as "Page break before" (Format
| Paragraph | Line and Page Breaks).
