If you mean to add to the QAT (Quick Access Toolbar) then you can't.
It is possible to edit the ribbon -
http://gregmaxey.mvps.org/Customize_Ribbon.htm and
http://ribboncustomizer.com/ to add icons of your choice, and if you still
have Word 2003 available - see
http://www.gmayor.com/Toolbars_in_word_2007.htm
