Some of the headings are not being shown when trying to insert a
cross-reference within the same document, say as a cross-reference or a
hyperlink. The heading is shown within the Table of Contents, somehow.

Any ideas?

I suppose you are referring to what you see if you select "Heading" as the
reference type in the Cross-reference dialog box. The list of items will only
include paragraphs that are formatted with the built-in Heading styles
(Heading 1 - 9). The problem could be that the missing "headings" have been
applied other styles.

A TOC can be set up to include any style, i.e. it can include items
formatted with other styles than the Heading styles.
