Can anyone tell me how to create a book with addresses inside with 4
wide across and 6 down? I want a to create an 8 1/2 x 11 page and fold
it in half to create the book.

Is there a template to use for this? Should I use some other program
for this?

In Page set up, under Pages: multiple pages, select bookfold. Word will set
up the pages for you. Then use a table to get the columns you require.
Remember that a booklet must be in multiples of 4 pages.
