Default Survey form with blanks to fill in for website?

I need to make a survey form for my website. What do I do to have one where
the person can fill in the blanks, but the blanks will not automatically get
any longer or take up another line if they keep typing? I want the blanks to
appear like this _____.

Use table cells with fixed dimensions.
