Default How do I auto insert text in multiple locations w/o the ASK box?

I have a form that I am putting TEXT FORM fields in that was previously a
non-fill environment. Each page asks for the persons NAME and DATE. I would
like all of the pages to fill with what is entered on page one so that they
do not need to fill in those particular blank(s) multiple times. I have tried
using the cross-reference to bookmark option, but I keep getting the ASK
field since it is set to 'fill-in on exit". How do I get the fields to fill
without this extra pop-up happening?
