Default TOC appears correct until printed, then some entries gain anot

I would expect combining styles and TC fields to cause problems if TC fields
were used to include headings that formatted with styles that are included
in the TOC. Whether it's possible to use just one or the other depends a lot
on the headings.

changed from the original, both the old page number and the new would

side by side. Try selecting the TOC and then accepting changes in it.
