Default How Do I Make ActiveX Go Away?

I began with a plain Word document and was entering form fields. I couldn't
find the old "Combo-Box" option on my Forms menu. As I tinkered around I
ended up opening the ActiveX Control ToolBox and now the lock feature on my
Forms toolbar is grayed out. Does Word think this is an ActiveX form now?
How to I make it go back to being a plain old Word doc with form fields?

There has never been a combo box form field; this is always an ActiveX
control.
