Default Make a statement of one incident I was witnessed

I was witness of one accident. For that I would like to make a statement for
insurance company.

The term is normally called an "accident report."
Whether Word has an existing template for such a thing, I do not know.

Generally speaking, the insurance company will typically either take an oral
statement from you, over the phone, or actually send an investigator out to
have you write one out. My suggestion: Ask the insurance company.
