Default allow comments in protected document

I am trying to create a form that has boxes that a person can put a check
mark. After they put the check mark they need to add a comment, but in order
to activate the box to allow a check mark I have to protect the sheet. When
I protect the sheet you cannot add comments to the sheet. The form is in a
table format.

There are a number of ways to create and use clickable checkboxes in
documents / templates. Some require protected or locked forms, others do
not. Take a look at the Checkbox template available at

You can add text form fields that will permit freeform comments.
