Default Word 2003 w/ Field entries

Word 2003 SP2
WinXP Pro SP 2

I've seen some Word documents which are created to allow entries into
selected fields. How can I do this? I'm not too sure what this is even
called to do an F1 search. Can I then export the results into an *.csv file
for import/analysis into Excel or another application?

I've used Word fairly heavily for several years but I've never 'tried' this?

Can I create character-based, numeric-based, currency-based, memo-based and
Boolean-based fields? Max/min lengths? Easy to setup and save as a template?
